Food & Exercise Tracking Form
The Full Story
The HartsSpace Food & Lifestyle tracker was made by HartsSpace provider, Michelle Benson. Michelle is a Certified Nutritionist with Washington State, specializing in Functional Nutrition and Eating Psychology/Behavior. She received her Master's Degree at University of Western States in Portland, Oregon where she received her Masters in Human Nutrition and Functional Medicine. She also completed an Eating Psychology Certification from the Institute of the Psychology of Eating in 2014. Armed with these two backgrounds, Michelle understands how daunting and tedious traditional tracking apps can be. In her work with patients, she found that uncovering various patterns were much better done with thoroughly tracked data, but not necessarily with a strong focus on portion size, unless the patient notes that they felt they overate or underrate at any given meal. This form has been given to patients to uncover things like IBS, dairy sensitivities, fructose malabsorption, binge eating triggers, and more. By submitting your data using this form and working with a HartsSpace provider, you can uncover what works well and what does not. This can be hard when just using recall information.
